If someone is replying to an anonymous post using their CB name (not replying anonymously), why does it have to be approved by moderators? Is it due to the limitations of the platform or something that can be changed?

Unfortunately that cannot be changed currently. Right now the ability to moderate posts is set at the forum / node level. This means that all posts added into the anonymous forum enter the moderation queue. I have been really testing out the permissions... but I don't think there is another way to do it. I will be testing it more though as this is a very new feature for the site.
