Sorry for the late reply. The view of all involved in treating her (doctor, physio and orthopedic surgeon) was that given the length of conservative treatment and the wide range of treatment tried, she would not have benefitted from cortisone. I hope it works for your daughter!

At our gym a coach stays outside with the kids until their parents fetch them. This is also for road safety and they allow older kids to walk to parents' cars as long as they can see the parent or whoever is allowed to pick up the child.
