Coaches Gymnast info sheets?

We don't normally take gymnast info when we get new people coming into the club, mostly because they are all adults, all students at the university, and we get many people who come for one session to try it and are never seen again. However, we are now under increasing pressure from the uni to show athlete progress and are starting to use a scheme loosely based on the BG award scheme but slightly more geared to grown-ups.

As part of it we need a form that newbies fill in to assess their current level and it makes sense to get things like their contact details and any medical conditions or injuries at the same time, but I feel like I'm missing stuff that probably vital!

So far I'm asking for:
Name
Birthdate
Email
Phone
Student ID number
Level of experience
Any qualifications they may hold (coach/judge/official)
Emergency contact name, phone and relationship.
Medical conditions or injuries we need to know about.

Am I missing anything or just being paranoid?!

Beam deductions

Hi everyone! I’m hoping to get some help understanding my deductions on beam. I’ve been watching my videos back, but I’m having trouble figuring out exactly where the judges are taking the most off. If anyone is willing to take a look and give me some feedback, I’d really appreciate it. I’m especially curious about execution deductions, rhythm/connection issues, and anything in my acro or dance that might be costing me. The judge started me at a 10 for this routine, and I got a 9.1
